Cryptococcosis is the disease caused by the fungus known as Cryptococcus. Cryptococcus is a type of fungus that is found in the soil, usually in association with bird droppings. The major species of Cryptococcus that causes illness in human is Cryptococcus neoformans, which is found worldwide. Another less common species that can also cause disease in humans is Cryptococcus gattii. Since 1999, C. gattii has also been found in regions of the Pacific Northwest, particularly in Vancouver Island in British Columbia, and Oregon and Washington in the United States.

There is patchy ill-defined T2 FLAIR hyperintense signal involving the subcortical, deep, and periventricular white matter of the supratentorial compartment, worse in the frontal horn and periatrial regions. There is a more nodular region of T2 FLAIR hyperintense signal within the subcortical white matter of the posterolateral left temporal lobe and patchy T2 FLAIR hyperintense signal abnormality in the substance of the midbrain tegmentum extending into the tectal plate. There are no suspicious abnormalities on the diffusion-weighted images, and post contrast sequences demonstrate ill-defined enhancement along the perivascular spaces worse in the bilateral frontal regions. A differential of meningoencephalitis, demyelinating disease, and vasculitis was given. Lumbar puncture and CSF fluid analysis was performed demonstrating a cryptococcal meningitis. CNS cryptococcosis results from infection of the central nervous system the with the fungus cryptococcus neoformans. Disease is more commonly seen in immunocompromised individuals. Clinical presentation typically begins with meningitis or meningoencephalitis, headache, seizure or altered vision due to raised intracranial pressure. Premature babies also have weakened immune systems, especially if they are underweight.

There are three types of meningitis this includes bacterial meningitis, viral meningitis and fungal meningitis. Bacterial meningitis, there are two types the pyogenic Bactria and tubercular bacteria leads to meningitis. Tubercular meningitis is very common in India, bacterial meningitis which is usually responsible for the meningitis or the meningococcal, streptococcal and pneumococcal. Viral meningitis is a very common it affect the any individual but it is less severe most of the time it will resolve on its own. Fungal meningitis is very severe form of meningitis usually it affects the people with a very low resistance so we have to rule out conditions like AIDS in this patients. Cryptococcus Neoformans is the capsulated which is usually involved in this condition, to diagnosis the meningitis after clinical evaluation we have to go for lumbar puncture and he CSF which we get from the lumbar puncture we have to analysis and we can come to the conclusion whether it is a bacterial or fungal meningitis. Accordingly we can treat the condition.

This raises a fundamental question in microbial pathogenesis: Why do some of these organisms cause disease in mammals? In this lecture we will dissect the biology of the human pathogenic fungus Cryptococcus neoformans in an effort to glean an explanation for the origin of virulence. C. neoformans is an intracellular pathogen with a remarkable replicative strategy that includes the capacity for exciting the host cell without triggering its lysis. This process involves inflicting just enough damage to the host cell to interfere with its microbicidal properties without triggering cell-death pathways such that intracellular replication can proceed unhindered while the host cell remains alive to participate in the exit of fungal cells. The similarity in the interactions between cryptococcal cells with macrophages and amoeba has led to the proposal that the cells' capacity for mammalian virulence emerged accidentally as a result of environmental interactions with phagocytic predators. In this lecture we will also explore the fascinating properties of melanin, an enigmatic pigment that performs a myriad of functions from enhancing cell-wall integrity to energy transduction. We will also consider some of the immunological lessons from studying C. neoformans, which produced important insights into novel antibody protective functions. Finally, we will look at the big picture of fungal pathogenesis and explore the concept of accidental virulence and the likelihood that global warming will bring new fungal diseases. Cryptococcosis is the disease caused by the fungus known as Cryptococcus. Cryptococcus is a type of fungus that is found in the soil, usually in association with bird droppings. The major species of Cryptococcus that causes illness in human is Cryptococcus neoformans, which is found worldwide. Another less common species that can also cause disease in humans is Cryptococcus gattii. Since 1999, C. gattii has also been found in regions of the Pacific Northwest, particularly in Vancouver Island in British Columbia, and Oregon and Washington in the United States.
